W20802.Safety Incident Handling Process


Purpose: To provide a process for reporting safety incidents.

Scope: Applies to all employees and contractors working at or for Duer/Carolina Coil.

Remarks: None.

Definitions:

  1. Incident: An occurrence that caused or could have caused an injury.
  2. Recordable: In accordance with applicable OSHA requirements, typically requires a physicians care and follow-up.
  3. Non-Recordable: Incident that requires only first aid or no treatment. Also applies to near misses.

Responsibilities:

  1. Employee: Report all incidents, regardless of severity.
  2. Supervisor: Document the incident, take immediate action to treat any injury and correct unsafe conditions.
  3. Manufacturing Manager: Investigate injury causes and determine further action needed to prevent future injuries.

Task Description:

  1. The employee must report any injury, regardless of severity, that occurs during work to their supervisor.
  2. The supervisor assists the employee in caring for the injury:

    1. Call 911 if the injury is severe and transporting the employee may cause potential further injury or harm.

    2. The Production Supervisor shall transport the employee to:

      1.     Duer/Carolina Coil Employee:

        1. Mary Black Urgent Care on Reidville Road in Spartanburg from 8:30 a.m. until 10 p.m.

        2. Spartanburg Regional Hospital at other times.

      2. If the employee is a temporary employee with Phillips Staffing:

        1. Call Tracey Lell at 864-574-8370.  If after hours, also call and leave a message at 864-354-4426

        2. Monday - Friday 8:00 am to 5:00 pm: Send the employee to Regional Occupational Health, 8311 Greenville Highway (Highway 29), Spartanburg, SC.

        3. Monday - Friday 8:00 am to 10:00 pm or Saturday - Sunday 8:00 am - 5:00 pm: Send the employee to Regional Westside Urgent Care, 8311 Greenville Highway (Highway 29), Spartanburg, SC.

        4. If the situation is an emergency or at other times than listed above, send the employee to the Spartanburg Regional Medical Center’s Emergency Room. 

        5. If unsure about the severity of the injury and the appropriate treatment, call the Regional On Call triage nurse at 864-577-4059.

    3. Call the Manufacturing Manager, regardless of time of day, if the employee is sent to an outside facility for care.

    4. Provide first aid, if qualified, to the employee. Typically, the employee should handle any of their own bodily fluids, if possible. This avoids contamination with blood borne pathogens.

  3. The supervisor completes the appropriate form:

    1. F10009.Non-Reportable Accident Report
    2. F10010.Reportable Accident Report
  4. The supervisor forwards the form to the Human Resources Assistant for record keeping.
  5. The Human Resources Assistant documents the incident of the OSHA 200 log and will complete any paperwork required by the temporary agency, if needed.
  6. The Manufacturing Manager works with the employee, any other employees involved, the supervisor and the Safety Committee to attempt to prevent future occurrences of the injury causes.
